Thank you for your response. Your idea relates to the Audit logs for Contact Center, Entity Type > Agent Routing Info events. The Audit logs for this entity are populated when the User updates Direct Routing settings under the User profile.
ACD Skills, when removed/added from the User, these events are logged into Audit under Service Directory -> User (see the screenshot):

What is still missing in the Audit logs is: Adding/Removing ACD Skill from the Org.

Is there a contact center skills Audit? The Audit API or the Audit log viewer don't seem to have this option. Are there any plans for a skills in audit log viewer?
