It usually means the campaign attempted to place the call, but the failure happened before the call could be completed, and the UI does not expose the root cause.
To troubleshoot, I'd recommend checking the following:
1) Outbound Event Viewer (Admin → Outbound → Event Viewer)
This is the first place to look. It often provides more specific error categories (capacity, callability, telephony, permissions, etc.).

2) Interaction / Conversation details
Find one of the failed attempts and check:
This helps identify whether the failure is happening at routing, agent, or carrier level.
