    I am unable to add image in Survey form header. Following the instructions but it is not giving the expected result. It is showing broken image icon. Need to know if i am missing anything?

    Hi Anshu,

    Have you followed the instructions here: Image formatting could you also confirm what file format the image is e.g. .png, .jpeg etc
