I had the same scenario as you, my agents had the required permissions but we were still getting errors.
I solved this after logging the agent, deselect the phone and then select the phone profile again. Hope this works for more people.

Manual dialing requires that a user be logged into the phone.
The user needs to have the Communicate User or PureCloud User role, or the conversation:call:add permission

Really I was very happy with the response to my last post by George.
When I was dialing a Number manually I am getting CALL ERROR
The initiator of the outgoing call does not have the correct dialing priviledge for the classification of call that is being dialed.
