Yes, you can find the form ID in GC. Navigate to the form:
Admin > Quality Admin > Evaluation Forms
Then click on the link to open the form you need the id for. Once you are in edit mode you can find the formID in the url bar. It'll be the final section. Here is an example from my org. I've bolded the formID.

Welcome to the forums. You can pull the information from an evaluation form using the API. https://developer.genesys.cloud/devapps/api-explorer#get-api-v2-quality-publishedforms-evaluations--formId-
You need to supply it the formID, but then you can pull the information, however it will be in JSON and will need to be parsed to get the information you want into other forms.
There is also an api endpoint to get info from an evaluation if you're wanting that. https://developer.genesys.cloud/devapps/api-explorer#get-api-v2-quality-conversations--conversationId--evaluations--evaluationId-
