I had the same issue last week. Are you also using agent groups to assign roles?
This bug has disabled the roles at the Group level (all groups) and removed the privileges of the agents (kicking them out of Purecloud).
Another inconvenient after reassigning the roles is that some agents were still getting the "Insufficient privileges" message. They had to reprovision their Softphones to make it work again.

Hello,
Is anyone else having issues where roles that have been assigned to a user somehow just vanish? I have had roughly 50 plus users that have been active over the last few days, go to log in today and get the error they dont have permission. When i check their profile, it shows Zero roles assigned.
Not sure if this is localized to my organization or not, thought i would check first.
