what would be the best way to monitor for outbound calls that are Emergency 911 with the notifications service. I reviewed all the topics but none jumped out as that could monitor new conversation and i wouldn't be able to use the user ID notification as that would pass the 1000 notification limit.

The only way I'm aware of would be to monitor every user's conversations as there is no way to monitor all conversations in the system. You can request new features and share your use case at https://genesyscloud.ideas.aha.io/.
