Talk about waking a sleeping thread :).
I looked back through my tickets and notes. The last entry i found was that on the SIP Proxy, we needed to set the Server Plan Distribution to 'Parallel' rather than 'Sequential'.

Some carriers don't send a SIP 180 after the session is setup (only a SIP 183). We have not been able to send ringback to the calling station on receipt of the SIP 183. Is there a setting that we can change to make this work?
