Hi everyone,
At present, when viewing an evaluation, it is not always easy for an Evaluator to understand why an evaluation has been created hence the QM team will surface the information about the source policy or person which created an evaluation. This will allow evaluators to understand why an evaluation has been created and sent to them. This information can be seen when you inspect the evaluation in Evaluator/Supervisor mode. In agent mode, the evaluation source information is unavailable.
Example:

If the name of the Policy or User is not retrievable, then it would show the following:
Created by Policy: Unknown
Created by User: Unknown
In instances where we're unable to trace back to the origin of the creator, the following will be shown:
Created by: Unknown
Note: Evaluations created before the implementation of evaluation source do not show this information.
