If i have outbound Campaign and make script to take Survey from customers, how Supervisor can collect all of this data in one report or where he can find this data

So with a script your agent can get the data into the script and I assume you are just looking for a way to get that data out of the script, correct? If so you have two ways to handle this. One you can use data actions to send the data out of the script to a place where you can store and correlate the data. Second you could use output variables in a script to store the values of the variables on the conversation: https://help.mypurecloud.com/articles/script-input-output-variables/.
